Avoiding readmissions: Payers and provider partnerships to improve care transitions

This webinar will examine how payers are working with hospitals and providers to avoid readmissions and improve care coordination across the continuum. Hospitals are now facing reimbursement cuts of up to 1 percent for Medicare patients who are readmitted within 30 days of discharge. This dreaded penalty can add up to seven figures in some cases. Most providers agree that intervention—from the start of initial hospital stay admission to discharge—encourages patients to adhere to medications and follow through on their primary care follow up. By managing the discharge process, hospitals can better communicate, not only with inpatient providers, but also with outpatient caregivers, such as skilled nursing facilities, community clinic and social services, thereby avoiding unnecessary readmissions.
